Disclaimer:  I do not read or understand Japanese.  
This being said, the jacket cover is a bit deceiving. The cover and title indicate a family of husband & wife, two daughters and a son who get stuck in an elevator and enjoying familial incest.  However, the time stuck in the elevator turns into days. Then, the son's mental health begins to deteriorate to the point where he holds the family hostage with his scissors and terrorizes them.

Personal commentary on the story-line:  this is good acting by the guy who plays the son.  Not everyone can carry off slow mental deterioration in a confined space.  As unbelievable as it is, he terrorizes his family with scissors.  Not sure if there is a cultural attribution here--Japan's sensibilities may treat first born sons differently even if mentally unstable.  In the US, if a terrorized family calls 911 on their son, chances are high that the police would kill the son--typically the son would be waving a kitchen knife or pocket knife or machete or hatchet.  The JAV idols did well in displaying terror--I'm sure this is from acting in rape story-lines, which seem standard for all JAV idols.

The sex scenes are relatively short:
  1) son forces elder sister to have sex with him;
  2) son forces younger sister to give father oral sex;
  3) son forces both sisters to engage in lesbian acts;
  4) son forces mother to masturbate....
  5) Without story-line development, after some sleep, the mother and two daughters seduce the son into sex.  To me, this was hurried.

Final scene shows repairmen opening the elevator door and seeing the entire family engaging in sex.  This is the scene on the cover jacket

I had not seen a JAV with such a story-line before, so watched more out of curiosity.
The director probably has aspirations of doing more serious cinema.
